FEES
SMART receives revenues through extended health care benefits, motor vehicle accident insurance or those willing to pay privately. It is not funded by OHIP. Fees are due upon day of service. No direct billing. Cash, Debit, Visa, Mastercard or American Express are preferred methods of payment. A receipt is issued for extended health care reimbursement at the end of your treatment plan.

Please come appropriately dressed and allow for 45 minutes to one hour.
Bring any test results or x-rays you may have.

Single Game Game Rate Tournament Rate $30/hour* $75/game* Full Day $300* $50/playing surface* Half Day $175*
* Rates for (1) one Certified Athletic Therapist covering a field event. Supplies, travel, extra coverage, etc., should be negotiated separately.
